Prenuvo vs Neko Health vs SimonMed vs Ezra: Full-Body MRI Compared (2026)

If you’ve priced out a full-body MRI recently, you’ve probably noticed two things. First, the range is enormous — anywhere from about $900 to $4,000 for what gets marketed as roughly the same promise: catch something before it catches you. Second, almost every “best full-body MRI” comparison you’ll find online is published by a company that sells one, or gets paid when you book one. Insurance won’t touch any of it, so you’re spending real money on a decision that most of the usual research shortcuts — reviews, rankings, referrals — aren’t actually built to help you make honestly.

This article is our attempt at that honest version. We compare Prenuvo, Neko Health, SimonMed Longevity, and Ezra on the same fixed criteria — price, what’s actually included, scan duration, locations, and whether each company has published anything to back up its claims — and we grade the evidence the same way we grade everything else on this site. We don’t sell scans, and we have no paid or affiliate relationship with any provider in this comparison.

Prenuvo

Prenuvo is the name most people mean when they say “full-body MRI.” Founded in Canada and now operating clinics across the U.S., Canada, and the U.K., it’s the largest dedicated player in this space by scan volume and the one with the most brand recognition, partly from high-profile users discussing it publicly.

Pricing. Prenuvo runs both a membership model and standalone scans. Membership tiers, billed annually, are Core ($1,199/year — a 45-minute focused scan covering head-to-mid-thigh, plus a core blood panel), Comprehensive ($2,499/year — a 60-minute whole-body scan plus an expanded panel and a 6-month repeat labs check), and Executive ($3,999/year starting, varying by location — a 75-minute scan adding body composition analysis and advanced brain/heart assessments in select markets, plus an executive blood panel). Standalone, non-membership scans run $1,199 (focused, ~30 min) to $2,499 (whole-body, ~50 min).

Scale. Prenuvo publicly confirmed over 100,000 scans performed as of a March 2026 interview with CEO Andrew Lacy on The Heart of Healthcare Podcast, following a 100,000-scan milestone the company announced in December 2024. Higher figures for total scans circulate in some secondary coverage; we were not able to independently confirm any number above 100,000 as of this writing, so we’re reporting the figure Prenuvo itself has most recently and publicly confirmed.

What they publish. This is where Prenuvo separates itself from the other three. The company has presented real-world data from its Polaris study — roughly 1,011 mostly asymptomatic participants — at the 2025 American Association for Cancer Research (AACR) Annual Meeting, reporting a 2.2% biopsy-confirmed cancer detection rate and a 99.8% negative predictive value (patients with a clear scan remaining cancer-free for at least a year), as covered by AthleTech News. Separately, Prenuvo has registered Project Hercules, a 10-year, 100,000-participant prospective study on whole-body MRI screening performance, run out of a dedicated research center in Watertown, Massachusetts.

What they don’t disclose. Neither Polaris nor Hercules has yet produced a completed, peer-reviewed outcomes paper — the kind that would show whether scanning changes survival, not just detection rates. A 2025 letter to the editor in European Radiology specifically pushed back on how cancer-detection utility figures from whole-body MRI programs are being interpreted and reported.

ScanGlean evidence grade: Grade B — real, company-sponsored prospective data exists and is more extensive than any competitor’s, but it has not yet cleared independent peer review with outcomes data, and the core claim that whole-body screening extends life for an average-risk adult remains unproven industry-wide.

Neko Health

Neko Health gets compared to Prenuvo constantly, and the comparison is misleading in one specific, important way: Neko Health does not perform MRI scans. Co-founded by Spotify’s Daniel Ek and Hjalmar Nilsonne, Neko uses a proprietary combination of high-resolution 3D and thermal cameras (skin and mole mapping), laser-based cardiovascular assessment paired with ECG, and a blood draw — all reviewed with you in person by a physician.

Pricing. £299 per scan in the U.K., a single flat price with no membership tier.

Scale and funding. Neko has raised aggressively: a $260 million Series B in January 2025 valued the company at $1.8 billion, TechCrunch reported at the time. On July 15, 2026, Neko announced a $700 million Series C led by Lightspeed Venture Partners and O.G. Venture Partners — with Mark Zuckerberg and Priscilla Chan joining as individual investors — at a valuation approaching $7 billion, explicitly to fund U.S. expansion. As of January 2025, Neko reported a waitlist of over 100,000 people and roughly 10,000 patients scanned to date across its Stockholm and London clinics; more current figures for either number were not publicly available as of this writing. Its first U.S. clinic, in New York City, is scheduled to open in fall 2026.

What they publish. Neko is running or has registered several component-level clinical studies, including the DermaFlow trial (ClinicalTrials.gov NCT06989216), evaluating the sensitivity and specificity of its skin-imaging technology for melanoma and other skin conditions, and the MIND study on vascular effects of diabetes. As of this writing, these trials are in progress; we did not find completed, published sensitivity/specificity results for Neko’s screening system as a whole.

What they don’t disclose. There is no published outcomes data — from Neko or anyone else — showing that this specific multi-sensor combination changes health outcomes at a population level, and because it is not FDA-cleared as a diagnostic device in the way individual imaging components sometimes are, it sits entirely in the “wellness screening” category rather than a regulated diagnostic one.

ScanGlean evidence grade: Grade C — the underlying components (ECG, dermoscopy-style imaging, standard blood panels) are individually well-established medical tools, but the combined proprietary system, as marketed for general screening, has no completed published validation we could locate.

SimonMed Longevity

SimonMed is not a scrappy startup — it’s one of the largest private radiology groups in the U.S., operating 170+ imaging locations. In January 2026, it launched SimonMed Longevity, a dedicated division offering tiered whole-body-adjacent MRI packages, explicitly aimed at the same buyer Prenuvo and Ezra are chasing. This is arguably the more important story in this space than any single new startup: a mainstream, insurance-network radiology chain is now selling elective preventive MRI at retail scale.

Pricing. Three tiers: Core MRI ($899 introductory / $999 list) covering chest, abdomen, and pelvis in about 30 minutes; Core MRI Plus Head and Neck ($1,599 / $1,799) adding brain and neck imaging in about 45 minutes; and Core MRI Plus Head, Neck and Spine ($2,199 / $2,499) adding a full spine and MRA in about 60 minutes. Bloodwork and DEXA/body composition are not bundled into any tier.

Scale. SimonMed Longevity launched at 30 locations and expanded to more than 70 sites by the end of Q1 2026, layered on top of SimonMed’s existing 170+-location diagnostic imaging footprint across ten states.

What they publish. SimonMed markets its protocol as built on 3T MRI technology (twice the field strength of standard 1.5T machines, producing more detailed images) with “AI-enabled” imaging support and reads from board-certified radiologists. 3T MRI itself is well-established, widely used diagnostic hardware — that part isn’t in question. What we could not find is any SimonMed-published data specific to outcomes from its Longevity screening protocol in asymptomatic adults — detection rates, false-positive rates, or follow-up statistics of the kind Prenuvo has at least partially presented.

What they don’t disclose. No published methodology or outcomes data specific to the Longevity screening program itself, despite the AI-enabled marketing language. All follow-up consultations are included, and reports arrive within 5–7 days via digital access.

ScanGlean evidence grade: Grade C — for the specific claim that this “AI-enabled longevity” packaging improves outcomes over a standard MRI read. The underlying MRI hardware and radiologist interpretation are standard-of-care tools with a long, separate evidence base for diagnostic (not asymptomatic screening) use.

Ezra

Ezra has gone through the most public repositioning of the four. Founded as an independent full-body MRI startup, it was acquired by Function Health — the biomarker-testing company co-founded by Dr. Mark Hyman — in May 2025, explicitly to combine lab testing with imaging under one brand, CNBC reported at the time.

Pricing. A base MRI scan (head, neck, abdomen, pelvis) runs $999, or $899 with a Function Health membership; adding spine imaging brings it to $1,699 ($1,499 with membership); the top tier, adding musculoskeletal and brain assessment plus body composition profiling across two roughly 60-minute sessions, runs $3,999. Function Health membership itself is $365/year and includes 160+ lab biomarkers reviewed twice a year. Ezra’s original, heavily promoted “$499 full-body MRI” from its May 2025 relaunch announcement was a limited-time introductory price and is no longer available.

Scale. Not independently disclosed in current reporting; exact patient volume for Ezra specifically (versus Function Health’s broader membership base) was not publicly available as of this writing.

What they publish. The one concrete, independently verifiable claim here: Ezra’s proprietary Ezra Flash AI image-processing model has received FDA 510(k) clearance, most recently expanded in early 2025 to cover head, abdomen, and pelvis MRI sequences. This clearance is specifically for image quality and noise reduction in faster MRI protocols — it is a real regulatory milestone, but it is a clearance for image-processing software, not an approval of the screening program’s diagnostic or outcomes benefit.

What they don’t disclose. No separate published study on cancer detection rates, false-positive rates, or outcomes specific to Ezra’s screening protocol was found. The company’s public claims center on scan speed and price, not clinical performance data.

ScanGlean evidence grade: Grade B for the Ezra Flash AI clearance specifically, since FDA 510(k) clearance is real and independently verifiable. Grade C for the broader claim that the screening program itself improves health outcomes — no published data on that exists yet, the same gap every provider in this article shares.

What Full-Body MRI Can — and Can’t — Do

This is the section every provider’s marketing page leaves thin, and it’s the reason a comparison like this exists at all.

The case for it is real. A whole-body MRI genuinely can find things a standard checkup won’t — aneurysms, certain early-stage tumors, structural abnormalities. Published detection rates for confirmed cancer in asymptomatic screened populations run roughly 1% to 2.2% across various studies, including Prenuvo’s own Polaris data. Peer-reviewed literature reports clinically significant incidental findings — abnormalities substantial enough to warrant follow-up — in a range of roughly 8% to 14% of scans, depending on the study and protocol. We were not able to independently verify a single specific “14.4%” figure sometimes cited for undetected significant findings including early cancers in this space; if you’ve seen that exact number attributed to a specific study, we’d genuinely like the source — email us at the address in our Methodology page.

The case against blanket screening is also real, and it just got a sharper edge. A September 2025 JAMA Internal Medicine analysis, led by researcher Vishal Patel, looked at the well-publicized rise in early-onset cancer diagnoses and found the increase is “more apparent than real” for several rapidly growing cancer types — driven largely by more intensive screening and diagnostic scrutiny rather than a genuine rise in disease. Eight cancer types with rapidly rising incidence since 1992 showed essentially flat aggregate mortality over the same period; breast cancer diagnoses rose in women under 50 while breast cancer mortality fell roughly 50% over the same window — the pattern you’d expect from better detection of things that were often never going to be lethal, not from a true epidemic.

That’s the overdiagnosis problem in a single dataset: finding more disease is not automatically the same thing as saving more lives, and it can carry real costs — unnecessary biopsies, follow-up scans, and a diagnosis (and treatment) for something that would never have caused harm. Radiologists at academic medical centers that don’t currently offer whole-body screening have said as much directly. Dr. Thomas Kwee, a radiologist at the University of Groningen whose research is among the most cited in this field, put it bluntly in an RSNA report: “Simply detecting more cancers does not mean the technique is beneficial.” Fred Hutchinson Cancer Center and UW Medicine, which do not offer whole-body MRI screening and won’t reinterpret outside scans, cite the same core issue: “long-term patient outcomes and cost-effectiveness are not fully evaluated.”

Both of those things are true about the same technology at the same time. A scan can genuinely catch something serious in one person and generate months of unnecessary worry and follow-up procedures in ten others — and from the outside, before you get your results, there’s no way to know in advance which person you’ll be. That tension, not a verdict either for or against scanning, is what an honest comparison actually has to sit with.

Who Should Actually Consider This

We’re not going to tell you whether to book a scan — that’s a decision for you and a physician who knows your actual history, not a comparison article. What we can do is hand you the questions worth asking before you spend $900 to $4,000 finding out.

  • Do I have a specific risk factor — a hereditary cancer syndrome (like Li-Fraumeni), a strong family history of a specific cancer, or another documented elevated risk — that changes the math on pre-test probability, or am I healthy with no particular reason to suspect something is wrong?
  • What would I actually do differently if this scan found something ambiguous — an indeterminate nodule, a cyst, a lesion that needs monitoring rather than immediate action? Am I prepared for that outcome, which is statistically more likely than either “all clear” or “confirmed serious disease”?
  • Am I choosing this instead of, or in addition to, my guideline-recommended screening — colonoscopy, mammography, low-dose CT lung screening, coronary calcium scoring if applicable? None of these four providers are a substitute for those.
  • What is my own tolerance for a false alarm, in terms of both money (follow-up imaging, specialist visits, possible biopsy) and the weeks or months of uncertainty that can come with it?
  • Would my physician recommend this specific test for someone with my exact age, sex, and risk profile if I weren’t paying for it directly? That’s a conversation worth having before you book, not after you get a result you don’t know how to interpret.

Cost Reality: What the Sticker Price Doesn’t Include

The number on each provider’s pricing page is the beginning of the bill, not the whole thing, for a meaningful share of people who get scanned.

  • Follow-up imaging. If a scan flags something indeterminate — the single most common outcome across the published literature, more common than either “all clear” or “confirmed serious finding” — the next step is usually a targeted follow-up scan (a dedicated MRI or ultrasound of just that area), which is billed separately and, because it’s often framed as diagnostic rather than elective, may or may not be covered by insurance depending on your plan and how it’s coded.
  • Specialist consultations. A finding that needs a second opinion — a urologist for a kidney finding, an endocrinologist for a thyroid nodule, an oncologist for anything suspicious — is a separate appointment, separate copay, and separate wait time, on top of the scan itself.
  • Biopsies and procedures. The minority of indeterminate findings that need tissue sampling to resolve carry their own separate cost and, for some biopsy sites, real procedural risk — not just money.
  • Annual repeat scans. Every membership model in this comparison is built around rescanning — Prenuvo’s tiers explicitly include 6-month or annual follow-up, and any provider’s real value proposition depends on tracking change over time, not a single scan. That means the honest cost of “trying this” isn’t the first invoice; it’s the first invoice times however many years you keep it up.
  • Time. Scan appointments run 30 minutes to two full hours across these four providers, and that’s before any follow-up appointments a finding might trigger.

None of this means the sticker price is deceptive — every provider we reviewed publishes their base pricing clearly, which is more than can be said for the wider longevity industry (a market industry researchers at Market Intelo estimate at roughly $11.08 billion in 2025, projected to reach $33.4 billion by 2034 — figures that vary meaningfully across industry research vendors and should be read as directional rather than precise). It means the honest cost of full-body screening is the base price plus a real, non-trivial probability of a second and third bill you didn’t budget for.

The Bottom Line

Prenuvo, Neko Health, SimonMed Longevity, and Ezra are solving overlapping but genuinely different problems at genuinely different price points. Prenuvo has the deepest published track record and the widest tier range. Neko Health isn’t actually competing on the same axis as the other three — it’s a faster, cheaper, non-MRI screening built for a different kind of buyer. SimonMed Longevity is the clearest signal that this category is going mainstream, backed by a 170+-location radiology chain rather than a venture-funded startup. Ezra, now under Function Health, is betting that bundling labs with imaging is the real value-add, not the scan alone.

What none of them have — as of July 2026 — is independent, peer-reviewed evidence that their specific screening protocol reduces cancer deaths or extends life in an average-risk, asymptomatic adult. That’s not a knock on any one of these four; it’s the current, honest state of the entire whole-body screening category, and it’s exactly the gap this comparison exists to be upfront about, rather than paper over with a “best overall” badge.

Frequently Asked Questions

Is a full-body MRI worth it?

It depends entirely on who you are. For someone with a known hereditary cancer syndrome, a strong family history, or specific risk factors, a clinician may genuinely recommend it. For the average healthy adult with no symptoms, the American College of Radiology says there is not enough evidence that whole-body MRI screening prolongs life or is cost-effective, and published detection rates for confirmed cancer in asymptomatic people are low — around 1-2% — against incidental-finding rates that are much higher. It’s a real trade-off, not a clear yes or no.

Does insurance cover Prenuvo, Ezra, SimonMed, or Neko Health?

Generally, no. All four are marketed as elective, self-pay screening, and none are recommended by USPSTF, ACR, or other guideline bodies as population-wide screening — the reason most U.S. insurance plans do not cover them. Some providers accept HSA/FSA funds. If a scan is ordered for a specific diagnostic reason by your physician rather than as elective screening, insurance coverage rules are different — ask your provider directly.

How much does Neko Health cost?

Are full-body MRIs safe?

MRI itself does not use ionizing radiation, unlike CT scans, and is generally considered physically safe for most people (exceptions include certain implants and severe claustrophobia). The safety debate in this space is not about the scan itself — it’s about downstream harm from false positives and incidental findings: unnecessary biopsies, follow-up scans, and anxiety over abnormalities that turn out to be harmless. What’s the difference between Prenuvo and Neko Health?

Prenuvo performs an actual MRI scan — magnetic resonance imaging of internal organs, brain, and spine. Neko Health does not use MRI; it uses a proprietary combination of 3D/thermal cameras, lasers, ECG, and a blood draw to assess skin, cardiovascular, and metabolic health. Which full-body MRI provider has the most published evidence?

What happens if my full-body MRI finds something?

You’ll typically be referred for follow-up — further imaging, a specialist consultation, or a biopsy — to determine whether the finding is clinically significant. Published research on whole-body MRI screening consistently finds that most incidental findings turn out to be benign, but the follow-up process itself carries real costs, time, and anxiety regardless of the outcome. Ask any provider upfront what their follow-up pathway looks like and what it costs beyond the initial scan.
