Coronary calcium scoring is a particularly good illustration of what “the evidence actually supports” looks like in practice. In the Multi-Ethnic Study of Atherosclerosis (MESA), a long-running NIH-funded cohort study, a CAC score directly quantifies plaque burden in the heart’s arteries and has repeatedly outperformed traditional risk calculators at reclassifying patients’ true cardiovascular risk. Roughly 40% of people classified as “intermediate risk” by standard calculators, but with a CAC score of zero, were reclassified into a lower-risk category — a result with real, guideline-recognized implications for whether someone starts a statin.

Lung cancer screening tells a similarly specific story: the benefit is real, but it’s real for a narrowly defined group — long-term heavy smokers aged 50 to 80 — not for the general population. Applied to the right patients, the USPSTF concluded the benefits (fewer lung cancer deaths) outweigh the harms (false positives, incidental findings, overdiagnosis, radiation exposure). Applied to a 35-year-old nonsmoker looking for “peace of mind,” the same test carries only the harms, with essentially none of the benefit.

Where the Evidence Is Thin

This is the part the marketing usually leaves out.

Whole-body MRI screening for the general population

The American College of Radiology’s 2023 statement on the subject is unusually blunt for a professional body. It says there “does not believe there is sufficient evidence to justify recommending total body screening for patients with no clinical symptoms,” and that “there is no documented evidence that total body screening is cost-efficient or effective in prolonging life.” Its worry is that these scans turn up “numerous non-specific findings that will not ultimately improve patients’ health but will result in unnecessary follow-up testing and procedures, as well as significant expense.”

The published research backs that caution up with real numbers. A 2019 systematic review in the Journal of Magnetic Resonance Imaging, covering more than 5,300 asymptomatic people across 12 studies, found a pooled rate of 32.1% for critical or indeterminate incidental findings. A good chunk of those “critical” findings turned out to be false positives once doctors looked closer, and almost no studies tracked what happened to people with a clear scan over the long run. A more recent 2025 meta-analysis of over 9,000 asymptomatic people, published in European Radiology, found a pooled confirmed-cancer detection rate of just 1.57%, alongside what the authors called “modest detection rates, frequent incidental findings, unstandardized protocols, and lack of long-term outcome or cost-effectiveness data.”

To put that in plain terms: for every roughly 60–90 people who pay for a whole-body MRI, about one will have a cancer found — and roughly a third will be sent down a path of follow-up scans, biopsies, or specialist visits for something that, most of the time, isn’t cancer at all. The ACR does carve out an exception for genuinely high-risk populations, such as people with Li-Fraumeni syndrome or other hereditary cancer syndromes, where the pre-test probability of disease is high enough to change the math substantially.

Biological age tests, used for individual decisions

Epigenetic clocks are one of the more genuinely interesting developments in aging research, and the population-level science behind them is real. Steve Horvath’s original 2013 clock showed that DNA methylation patterns could estimate age with remarkable accuracy across tissue types. Since then, “second-generation” clocks have gotten better at predicting things that matter: GrimAge, built by researcher Morgan Levine and colleagues specifically to predict lifespan and healthspan, has consistently outperformed earlier clocks at predicting all-cause mortality in cohort studies. DunedinPACE, which grew out of the decades-long Dunedin Study led by Daniel Belsky and colleagues, measures the pace of aging rather than a fixed age, and has since been validated across more than 65 cohorts internationally.

Here’s the catch: all of that validation happened at the population level — comparing group averages across thousands of people over years or decades. Using a single test result to tell one individual person something actionable about their own aging trajectory is a different, much harder problem. A 2025 analysis in Epigenomics put it plainly: “fundamental technical and biological properties” of these algorithms — including sample handling, lab-to-lab variability, and tissue-specific differences in methylation — “prohibit their current use at the individual level.” The authors go further, warning that applying clock results to individual decisions “can be uninformative and potentially harmful.”

In other words: a biological age test can tell a research population something true and useful about aging on average. It’s a much bigger leap for it to tell you something precise enough to justify changing your medications, your supplement stack, or your anxiety level about your own mortality — at least with the current generation of tests.

“Check everything” general health checkups

This isn’t a new question, and it’s already been studied at a scale most people don’t realize. A 2019 Cochrane review — one of the most rigorous forms of evidence synthesis in medicine — pooled 17 randomized controlled trials covering more than 251,000 people to ask a simple question: do general health checks (a mix of questionnaires, exams, blood tests, and screening) reduce your risk of dying? The result: health checks had “little or no effect on total mortality” (risk ratio 1.00), cancer mortality (risk ratio 1.01), or cardiovascular mortality (risk ratio 1.05) — all rated high- or moderate-certainty evidence. The reviewers didn’t hedge: “general health checks are unlikely to be beneficial,” and may lead to unnecessary tests and treatment.

That finding is specifically about unfocused, general checkups — not about the individually targeted, evidence-backed tests listed in the section above. Why “More Data” Doesn’t Always Mean Better Health

None of this is really about people being gullible. The pull toward comprehensive scanning makes a lot of psychological sense, and it’s worth naming honestly.

Loss aversion is stronger than statistics. The fear of missing a treatable cancer looms much larger in most people’s minds than the diffuse, delayed cost of chasing down a false alarm. A scan that finds nothing feels like “proof” of health; a scan you didn’t get feels like an unquantified risk you’re carrying around. That asymmetry is exactly why marketing for these products leans so heavily on stories of scans that caught something, and so lightly on the base rates.

Modern primary care often doesn’t give people room to ask “what about my future risk?” A rushed annual physical, focused on acute complaints, can leave health-conscious people feeling like nobody is looking at the whole picture. Longevity clinics and full-body scans are, in part, a response to that gap — offering time, attention, and a feeling of being taken seriously that a fifteen-minute visit often can’t.

“More information” feels like it can only help. This is the intuition preventive-scan marketing leans on hardest, and it’s the one the research most directly contradicts. Every additional test carries its own false-positive rate, and every false positive can trigger what researchers call a “cascade of care” — a chain of follow-up scans, biopsies, or procedures with its own costs, risks, and psychological toll, often for a finding (an incidentaloma) that would never have caused you a symptom in your lifetime.

That’s the well-documented phenomenon of overdiagnosis: finding a real abnormality that would never have gone on to cause harm, but that you now have to live with, worry about, and possibly treat unnecessarily. Screening only works in your favor when the disease being looked for is both serious and detectable early enough to change the outcome — which is exactly why the evidence-backed tests above are narrow and specific, not broad and universal.

None of this means preventive testing is a bad idea. It means the decision to test is a trade-off with real numbers attached to it, not a costless act of virtue — and that trade-off looks completely different for a coronary calcium score in a 55-year-old with a family history of heart disease than it does for a whole-body MRI in a healthy 30-year-old with no symptoms.

How to Evaluate a Scan or Longevity Clinic Before You Pay

A few concrete questions can cut through most of the marketing:

  • What’s the false-positive and incidental-finding rate for this specific test? A credible provider should be able to point you to published numbers, not just testimonials.
  • What happens after an abnormal result? Who reads it, who explains it to you, and is a follow-up plan (and its cost) part of what you’re paying for — or a separate bill?
  • Is this test recommended for someone with my specific age, sex, and risk factors — or is it being marketed as universally beneficial regardless of who you are?
  • Is the protocol standardized? Ask whether the imaging protocol, blood panel methodology, or algorithm has been published or peer-reviewed, or whether it’s proprietary and unverifiable.
  • How is the provider paid, and does that affect what they recommend? Affiliate relationships and referral fees aren’t automatically disqualifying, but they should be disclosed.
  • Would a clinician recommend this test to someone with your exact profile if you weren’t paying out of pocket for a premium package? If the honest answer is “probably not,” that’s useful information.
